Stopping VAP Using The AACN Practice Alert

Purpose: The Intensive Care Unit (ICU) Staff at Queen of the Valley Hospital changed their practices around oral care and head of bed elevation following the publication of the AACN Practice Alert on Ventilator Associated Pneumonia (VAP). The goal of implementation was to decrease the VAP rate.

Description: Trending data demonstrated an increase in the VAP rate for the ICU population over the last 3 years. The ICU leadership team decided to implement the AACN practice standards noted in the VAP Alert. All RN staff received a copy of the practice alert for VAP at a staff meeting and biannual skills fair. Practice standards (head of the bed elevation, oral care) and the impact on VAP was reviewed with RN and respiratory care staff. Quarterly audits of nursing practice demonstrated a 95% compliance rate with the implemented standards.


Evaluation/Outcomes: VAP rates for intensive care patients dropped by 42% after the implementation of the head of the bed elevation and regular oral care. RN and respiratory care staff will continue to work collaboratively promoting these two initiatives to reduce the risk of ventilator associated pneumonia. The critical care leadership will continue to monitor the literature and look for additional interventions to reduce the VAP rate.
