Implementing FISH In Critical Care; Choose Your Attitude

Purpose: As a 16 bed, level I Medical Surgical Intensive Care Unit, it was clearly evident that there was a need to increase morale, overall job satisfaction, and improve the cohesiveness among all staff members. At the lowest point, we had greater than a 25% RN vacancy with a lack of a unit manager/director. A variety of measures were put into place in order to improve the overall status of the unit.

Description: The lack of staff cohesiveness was evident. We were unaware that senior management was already looking at network-wide morale improvement efforts. The unit was expected to implement the “FISH” philosophy. Through inservicing, individual book review and the formation of a unit based committee we were able to successfully implement this modern approach to job satisfaction. We based our goals on the idea of finding ways to improve the overall work environment, encourage team work and support of one another and thus decreasing the stress load of working in an intensive care unit. We achieved this through the direction of the unit-based Reward and Recognition Committee. The committee planned a Christmas party and summer picnic—both successful events. Encouragement of positive thinking sparked our unit’s staff members to form a “FISH” Committee. The committee decorates the unit seasonally. A bulletin board that highlights hire anniversary dates was started. We also have food days for holidays and birthdays. To brighten our weekend days, a tablecloth adorns the table in our lounge and sometimes even flowers are used for a center piece.

Evaluations and Outcome: As you can see implementing such an idea in a critical care setting is not easy undertaking. Throughout this past year we have met the challenge through the utilization of a staff satisfaction survey have clearly been successful producing positive outcomes as evidenced by an overall higher morale, improved work environment, and a supportive bridge between staff and management.
